Purpose: Mid-Term Evaluation of the Diabetes Prevention and Care Improvement Project

Purpose: The purpose of the assignment is to conduct a mid-term evaluation for the Diabetes Prevention and Care Improvement Project and propose recommendations for future directions.
Description:
Welfare Foundation is a not-for-profit, non-governmental organization staffed by a group of Georgian professionals holding postgraduate degrees in various areas of social sciences from some of the leading schools in Western Europe and the USA. The Organization focuses on the most sensitive areas for the vulnerable population such as health care, poverty and social welfare. One of the ongoing projects is "Diabetes Prevention and Care Improvement". The main goal of the project is to improve prevention and care of Diabetes in targeted areas (Tbilisi, Samegrelo, Adjara, and Shida Kartli regions of Georgia).
Scope of Work:
An evaluator is needed to conduct midterm evaluation in order to assess achieved results against the project indicators and budget targets, and its impact on direct and indirect beneficiaries. The evaluator will look at the progress and outcomes of the project that will be measured by qualitative and quantitative indicators such as:
** Number of Diabetes Patient Centers established;
** Number of patient support groups organized;
** Number of persons trained by the project;
** Number of planned and completed activities;
** Amount of funds spent on material/ human resources requirements;
** Number of fundraising/advocacy events organized;
** Number of informational resources produced and distributed.
Duties and responsibilities:
** To conduct and coordinate a wide range evaluation and analysis in order to assist in meeting established program goals and objectives.
** To collect and analyze quantitative and qualitative data to determine the effectiveness of program processes in meeting established goals and objectives.
** To perform meetings with a wide variety of management and staff personnel, both within and outside the program.
** To prepare reports on evaluation results and develop recommendations to correct deficiencies and improve program effectiveness.
** Other, as required by individual scope of work.
Requirements:
** Two or more years experience in the evaluation of large projects and/or non-profit programming with increasing levels of responsibilities, preferably in the public heath scope.
** Advanced degree (Master's or above) or equivalent in public health, health management or other related fields of social sciences;
** Ability to work as part of a team to plan and conduct evaluations.
** Ability to organize and summarize information in a clear and concise manner.
** Formal training in program evaluation processes, experience in similar program development/systems analysis activities.
** Exhibit professional and collaborative leadership skills
** Excellent oral and written communication skills in English.
Project details:
Goal:
Diabetes prevention and diabetes care to be improved in targeted areas (Tbilisi, Samegrelo, and Shida Kartli and Adjara regions) and among targeted populations (approximately 62 thousand people). The proposed project is focused on people living with diabetes (PLWD), type 1 and type 2, children and adults, their family members and caregivers (healthcare providers, social workers, home healthcare providers). The general population is a target group for raising public awareness about diabetes, its primary prevention, and healthy lifestyle improvement.
Objectives:
1. Establish nine Diabetes Patient Centres in three target regions in Georgia and develop an educational program (ToT model), materials, and tools for people living with diabetes (PLWD), their family members, and caregivers.
2. Establish educational program/resources for primary care physicians working in this targeted area for diabetes prevention and care improvement.
3. Strengthen organizational capacity of diabetes patient associations/ patient support groups in targeted areas to achieve their long-term sustainability (improve their leadership, advocacy, fundraising skills, and organizational resources).
4. Improved PLWD's navigation process related to their access to primary and secondary healthcare services/ medications for the targeted population.
5. Raise public and key stakeholders' awareness about diabetes prevention and issues related to PLWD's healthcare/social resources and rights.
Intended results:
As a result of the project activities, educational programs for PLWD, their family members, and caregivers (with about 1100 patients trained/year), various materials, tools, and patient support groups for self-care would be made available in nine new Diabetes Patient Centers.
More than 2000 PLWDs and their family members would be trained in self-care (how to monitor blood glucose, how to eat, how to plan physical activities). They would be better informed about available healthcare resources, insurance coverage, and mitigation services through patient navigation systems. Overall, PLWD would know how to access needed resources to control diabetes, prevent complications, protect their rights and live better lives. They would be able to pursue the same economic and social opportunities available to healthy Georgians.
By the end of this project 160 primary care physicians would be trained and equipped to prevent and manage diabetes, and prevent its complications. Diabetes prevention and care based on current evidence-based standards would be broadly available for primary care providers in Georgia. Diabetes Care Quality Metrics would be developed in order to measure current quality of care and identify its future needs.
Diabetes patient organizations would be able to develop their sustainable organizational capacity. They would be able to present key issues to the public and key stakeholders and be able to voice and protect their rights. They will be able to support their organizations through regular fundraising activities, patient empowerment and social events.
